What Does Intake Look Like at CMS?

New patients to CMS may or may not be in active withdrawal to begin treatment. During intake, we want to make sure that you are as comfortable as possible. Please let any CMS staff member know what you need during this time and we will try to accommodate you as best we can.

It may seem as though we are asking a million questions … we are. We want to know all necessary information to ensure the best care possible for you. Staff at Community Medical Services are dedicated to promoting a judgement-free environment so please answer any questions to the best of your ability. We are here to help you.

New to CMS?

Intake Criteria for Treatment at CMS

CMS is an opioid treatment program, or OTP for short. Because we are an OTP, our intake criteria are determined by various laws and regulatory bodies that OTPs are required to follow. Have you had problems related to opioids? If so, you are eligible for an intake screening at CMS.

Federal admission requirements state that any individual who meets diagnostic criteria for moderate to severe OUD, has an active addiction, is in remission, or is at risk of relapse or overdose is eligible for intake. If your problems related to opioids have been less than 12 months, but you were released from a correctional facility in the last 6 months, were treated for OUD in the last 2 years, inject any opioids or illicit drugs, or think you are pregnant, then you are still eligible for an intake screening at CMS.

Walk-ins always welcome! After a brief screening with a peer support or counselor, you’ll review medical history with a provider, receive your dose of medication, and be on your way. Be sure to bring a valid ID to your first visit.

Examples: Physician’s name, date of last appointment, practice/hospital name

To help speed up your intake process, we ask that our patients fill out, print, and bring two forms with them to their first appointment at a CMS clinic. This includes a demographic sheet and a language assistance acknowledgement.

Medically Assisted Treatment Programs
