What to Bring to Your Intake at Community Medical Services

Whether you’ve been referred to CMS or want to walk in and set up your treatment plan, you’re always welcome. After a brief screening with a peer support or counselor, you’ll review medical history with a provider, receive your dose of medication, and be on your way. Be sure to bring a valid ID to your first visit.
