This Policy does not apply to protected health information. Information you provide through the Services is not protected under confidentiality laws that protect health information. Any reference to Community Medical Services includes our affiliates, subsidiaries, officers, directors, shareholders, employees, and agents. Any first-person pronouns mean Community Medical Services.

The Services are provided “as is” and use of the Services is at your own risk. We cannot guarantee that the Services will be uninterrupted, error-free, or free of viruses, worms, Trojan horses, harmful or malicious code, or any other defects or issues. LAW AND VENUE
You agree that the laws of the State of Arizona apply to this Policy and your use of the Services without regard to Arizona’s conflict of laws principles. User irrevocably and unconditionally consents to the exclusive jurisdiction of the Superior Court of the State of Arizona located in Maricopa County, Arizona.
