The 2025 Patient Outcomes Report provides a nationwide view of CMS’s impact over the previous calendar year. Across the communities CMS serves, our mission is bigger than access to medication alone. It is about improving the social determinants of health that influence long-term recovery, including housing stability, employment, health access, community connection, and overall well-being.

Recovery is a long-term process, and meaningful outcomes often build over time. That is why CMS looks beyond a single appointment or test result to measure patient engagement, retention in care, changes in substance use trends, and improvements in the social factors that affect health.
Thirty-day retention is one of the most important early indicators of treatment engagement. When patients remain connected to care during the first month, they have more opportunities to receive medication support, counseling, harm reduction resources, and connection to services that may help them stabilize.
National urinalysis trends also help CMS identify shifts in the drug supply, monitor emerging risks, and support clinical decision-making. These data points allow CMS teams to respond with evidence-based care, patient education, and timely intervention.
State-specific outcomes reports were completed this year not for every state in which we provide care, but for those with the highest level of statistical significance. If you do not see your state below, refer to the national report for average data company-wide.

Screening is an important part of whole-person care. By identifying health needs earlier, CMS can help connect patients to education, follow-up care, and support that may otherwise be delayed or missed.

Weekly dosing data shows how patients progress through care, how treatment plans evolve over time, and how CMS supports individualized treatment based on clinical need.

Community Medical Services is amazing. They helped me with my substance abuse issue and got me back on the road to recovery. Methadone is only one of the tools you will need to help implement and maintain a successful sobriety strategy. This clinic, and my counselor, made sure that I had all the necessary tools to help me live a life clean and sober.
Been coming to see the very friendly, helpful, just really supportive staff. They do care about their work & the positive effects CMS has in the community. It’s 100% more than ok to ask for help and when you need it I’d definitely recommend them. They aren’t trying to give a short term solution, they want to help you build back a life you love.

CMS is the only way to go when you’re looking for recovery. I’ve been at other recovery centers and have never been successful with my sobriety as I have been now. The one and only reason…the staff. You will not find another center like this one! I am living drug free now for 2 years because of the amazing team at CMS!
